XV Fit Bootcamp — Liability Waiver & Full Disclosure

By registering for XV Fit Bootcamp, I ("Participant") acknowledge and agree to the following:

1. Nature of Activity

I understand that XV Fit Bootcamp involves vigorous physical exercise, group fitness training, and physical conditioning activities. These activities carry inherent risks including but not limited to: muscle strains, sprains, fractures, cardiovascular events, and other physical injuries.

2. Voluntary Participation

I voluntarily choose to participate in XV Fit Bootcamp and assume full responsibility for any risks, injuries, or damages that may occur as a result of my participation.

3. Medical Clearance

I confirm that I am in adequate physical condition to participate. I have either consulted with a physician prior to participation or I confirm that I do not require medical clearance. I agree to immediately inform a coach if I experience any pain, discomfort, dizziness, or shortness of breath.

4. Release of Liability

I hereby release, waive, discharge, and hold harmless XCLUSIV LIFE, XV Fit, its coaches, instructors, volunteers, staff, and organizers from any and all claims, demands, losses, damages, or liabilities arising from or related to my participation in any bootcamp session, including claims for negligence.

5. Rules & Safety

I agree to follow all safety guidelines, instructions, and directions provided by coaches during sessions. I understand that failure to comply may result in removal from the program.

6. Media Release

I grant XV Fit permission to use photos or videos taken during sessions for promotional purposes on social media and marketing materials, unless I explicitly opt out in writing.

7. Acknowledgment

I have read this waiver in full, understand its contents, and sign it freely and voluntarily. I am 18 years of age or older, or have obtained parental/guardian consent.
